Care Coordinator – Job Description

The Care Coordinator is responsible for outbound patient calls to guide, support, and drive conversion for preventive health services.

Key Responsibilities

• Make outbound calls to patients for engagement and service coordination.
• Sell and promote preventive health services including health check packages, follow-up tests, pharmacy services, and doctor consultations.
• Encourage patients to complete recommended screenings and follow-ups based on their health reports.
• Book appointments for consultations, health checks, and pharmacy support.
• Explain basic health report findings using simple medical terminology.
• Maintain call records, patient interactions, and conversion status in the system.
• Coordinate with internal teams to ensure smooth service delivery.

Requirements

• Strong sales experience (mandatory), preferably in call-based sales.
• Basic understanding of medical terminology.
• Preferred qualification: Allied Health Sciences / Life Sciences graduate.
• Excellent communication, persuasion, and patient engagement skills.
